Running Records in Kindergarten

If a student is reading the same word incorrectly throughout the book, is that word counted as incorrect every time or just once?

Or can the teacher tell the word and count it as one error and hope that the student will remember the word when it shows up again.

(Most level one books are have repetitive phrases).
